The plague spread through place to place by trade. Cities near the water got hit first which made the lose of people even higher because cities near water were the most populated.

Step #1. Black rats are infected with bacteria called Yersinia Pestis.
Step #2. Fleas bite infected rats.
Step #3. The rats die from the infection.
Step #4. Fleas abandon the dead rats and search for a new host.
Step #5. Humans are infected through flea bites and they die.
Step #6. Fleas abandon the dead humans and rats to find more hosts to infect.

According to Encyclopaedia Britannica an estimated 30% to 60% of the people in Europe have died because of the black death and 75 million deaths total. Other names for the Black Death were "Great Pestilence",  "Great Plague",or  "Great Mortality." The term "Black Death" got its name because of the late-stage sign of the disease, where the sufferer's skin would blacken due to hemorrhages.
